This publication is intended to build upon the requirements of event investigation set forth in the IAEA Safety Standards Series No. NS-R-2, Safety of Nuclear Power Plants: Operation, and also complements the IAEA PROSPER Guidelines — Guidelines for Peer Review and for Plant Self-assessment of Operational Experience Feedback Process. It contains best practices regarding the investigation of events to ensure that it is effectively organized and conducted to determine the direct causes, root causes, contributing factors and lessons learned to improve safety and prevent recurrence of events. Use of the guidance provided by this publication will be helpful to nuclear facilities and related institutions to strengthen and enhance their feedback process through the implementation of best practices in the organization, management and conduct of an effective investigation of events.
