An accidental overexposure occurred in the Bialystok Oncology Centre which affected five patients undergoing radiotherapy. This report gives an account of the event, the subsequent dose assessment and the clinical consequences for the patients. It also discusses the lessons learned and provides recommendations for preventing similar events from occurring. As such the report is likely to be of use to the manufacturers and users of accelerators, and to national bodies.
